New Client Intake

Guelph Community Health Centre (Guelph CHC) client intake is open with limited intake capacity to our ongoing waitlist.

Guelph CHC is specifically funded to provide health services to Guelph residents in the following priority populations. Please ensure you meet a minimum of two of these requirements plus have no current access to primary care. Previous care at another Community Health Centre does not automatically mean you meet our priority population requirements.

  • Homeless or at risk of homelessness/ vulnerably housed

  • Low income (check here of the latest income table) 

  • Newcomers to Canada (in the last 5 years) with language barriers

  • Moderate to severe mental health and/or addiction issue

  • Moderate to severe disabilities

  • Aboriginal communities, including First Nations, Inuit and Métis

  • 2SLGBTQI+ community members

  • Vulnerable children and their families experiencing violence, family conflict, social isolation and attachment struggles

  • Other marginalized groups

If you feel you meet the population served at Guelph CHC, please complete the intake application at the link below or drop by a Guelph CHC clinic to pick up a paper copy.  One application per person.  Please complete one application for each family member.

If you require support in completing an intake application for any reason, please call our Downtown clinic at 519-821-6638 Ext 1, when prompted, press 5 or call our Shelldale clinic at 519-821-6638 Ext 2, and leave a message with your contact details.  Please provide 7-10 days for a return call.
