New Client Intake

NOTICE: We are temporarily closing new client intake for our primary health care clinics downtown and at Shelldale, effective immediately (February 12, 2025).  

We are currently unable to assign community members to our primary providers in a timely manner, due to the large number of applicants on our waitlist. Instead, we will be spending the next few months reducing our waitlist and plan to reassess in fall 2025. Our apologies for any inconvenience this may cause. We look forward to reopening new client intake and connecting with you soon.

Please note, Guelph CHC is specifically funded to provide health services to Guelph residents in the following priority populations.

  • Homeless or at risk of homelessness/ vulnerably housed

  • Low income (check here of the latest income table) 

  • Newcomers to Canada (in the last 5 years) with language barriers

  • Moderate to severe mental health and/or addiction issue

  • Moderate to severe disabilities

  • Aboriginal communities, including First Nations, Inuit and Métis

  • 2SLGBTQI+ community members

  • Vulnerable children and their families experiencing violence, family conflict, social isolation and attachment struggles

  • Other marginalized groups
