What is the HART Hub?

Homelessness and Addiction Recovery Treatment (HART) Hub Guelph Wellington is a health care facility, and a regional network of care teams, that provides care, housing, and treatment for people in our community with the most complex needs. This includes people experiencing complex mental health and/or addiction challenges, housing instability or homelessness.

The HART Hub plans to bring support from different care providers together through a one-team approach. This allows us to offer care that responds to a client’s mental, physical, and substance use health needs, and meet them where they are on their journey. No matter where a client goes for support, we will be able to connect them with the care they need.

Who is the HART Hub for?

HART Hub Guelph Wellington provides supports for people in our community who:

  • Face barriers to accessing health services.
  • Have multiple complex health conditions, including moderate to severe mental health or substance use disorders.
  • Are homeless, precariously housed, or need intensive support to maintain stable housing.
  • Require highly specialized services from multiple providers to improve their well-being.

An estimated 2,000 adults and youth in Guelph and Wellington County are facing these struggles.

What is coming later in 2025-26

  •  Short-term, voluntary, non-medical residential beds offered to people ages 18+ experiencing a mental health or substance use crisis

  • 24/7 medical and social support for individuals who are withdrawing from substances.
  • these beds will not open until 2026-27
  • Integrated Crisis Centre: support and intervention for mental health and/or substance use crises (CMHA)

Mobile mental health and addictions care for residents of supportive housing sites in Guelph and Wellington County, to help them live independently and stay housed. *HART Hub supportive housing is not yet available

Notice from Guelph CHC: services as of April 1

  • On April 1, 2025, Guelph CHC’s CTS transitioned to Guelph Wellington HART Hub. Our staff team at Guelph CHC is the same. We still offer health care but can no longer offer supervised consumption services (CTS) or the Alternative Prescribing Program (APP). Our staff have been preparing for this, including decreasing CTS hours and winding down the APP program. 
  • Individuals and businesses, especially downtown, are already seeing more public substance use. Without a CTS we are not able to provide the level of monitoring we have been providing.
  • Other components of the HART Hub, such as Withdrawal Management & Crisis Stabilization (see below), are not yet available.
