What is the HART Hub?
Homelessness and Addiction Recovery Treatment (HART) Hub Guelph Wellington is a health care facility, and a regional network of care teams, that provides care, housing, and treatment for people in our community with the most complex needs. This includes people experiencing complex mental health and/or addiction challenges, housing instability or homelessness.
The HART Hub plans to bring support from different care providers together through a one-team approach. This allows us to offer care that responds to a client’s mental, physical, and substance use health needs, and meet them where they are on their journey. No matter where a client goes for support, we will be able to connect them with the care they need.

Who is the HART Hub for?
HART Hub Guelph Wellington provides supports for people in our community who:
- Face barriers to accessing health services.
- Have multiple complex health conditions, including moderate to severe mental health or substance use disorders.
- Are homeless, precariously housed, or need intensive support to maintain stable housing.
- Require highly specialized services from multiple providers to improve their well-being.
An estimated 2,000 adults and youth in Guelph and Wellington County are facing these struggles.
