All Feedback will be viewed as:

  • An opportunity to improve the work we do here at the Guelph CHC

What clients need to know:

  • You have the right to provide feedback and/or make a complaint if you are feeling unsatisfied with your experience at Guelph CHC.
  • You are encouraged to address the person with whom you are dissatisfied directly or contact the Clinical Services Manager.
  • Guelph CHC aims to address your feedback within 5 working days.

To provide your feedback:

  • Contact Thomas Wright, Clinical Services Manager at 519-821-6638 Ext.407.
  • Feedback can also be shared in writing, by email to info@guelphchc.ca, or in person by contacting the Clinical Services Manager to request a meeting
  • Your feedback will be directed to and supported by the most appropriate manager at Guelph CHC.
