To start your booking, you will need:
- Your Health card number
- Your provider’s name
If you do not know the name of your doctor or nurse practitioner, or do not know your/ have a health card number, please call the clinic at 519-821-6638.
To get started, have your health card number ready and click the location for your booking.
You can currently book online with your provider (doctor or nurse practitioner) for:
- Medication review
- Diabetes follow up
- General non-urgent follow up
Please note:
Online appointments are NOT for matters that require urgent, same day or same week care. If you need an appointment for an urgent matter or for something that is not listed above, please call the clinic at 519-821-6638.
If you require emergency care, please visit your nearest emergency department, or call 911.
