How do I become a client?

Guelph Community Health Centre is accepting limited applications for new clients at this time.

Please call the New Client Intake Coordinator at 519-821-6638 Ext 404 for more information, or if you would like to discuss resources for finding a family doctor in Guelph. 

Guelph Community Health Centre is specifically funded to provide health services to Guelph residents in the following priority populations. Please ensure you meet a minimum of two of these requirements plus have no current access to primary care.

  • Homeless or at risk of homeless
  • Low income (check here of the latest income table) https://www150.statcan.gc.ca/t1/tbl1/en/tv.action?pid=1110024101
  • Newcomers (in the last 5 years) to Canada with language barriers
  • Moderate to severe mental health and/or addiction issue
  • Moderate to severe disabilitie
  • Aboriginal communities, including First Nations, Inuit and Métis
  • LGBTQ2S+ populations
  • Vulnerable children and their families experiencing violence, family conflict, social isolation and attachment struggles
  • Other marginalized groups
