Community Health Connect Program is a new initiative of the Guelph CHC staffed by 12+ trained volunteers who assist clients by connecting them with the resources they need to be healthy. We are aware that our clients often face multiple challenges in their life, including limited access to food, unemployment, homelessness, social isolation, difficulty navigating the system and many other social determinant of health factors.
Volunteers greet clients and community members with a warm smile, offer them coffee, and ask if they require any kind of support and assistance. They listen empathetically to clients’ needs and then connect them to the community based resources.
Volunteers also help to complete forms and paper work, make phone calls to refer clients, provide directions, and follow up with clients to ensure their needs are met.
Community Health Connect Program hours of operation:
Monday/Wednesday: 12:30 – 4:30 pm
Tuesday: 9:00 am – 6:30 pm
Thursday/Friday: 9:00 am – 12:30 pm
